• THE PURPOSE OF HEALTH SCREENS

     

    The purpose of offering health screens is for everyone to receive baseline information on their current health status.  The screens will require a 14-hour fasting blood draw that will give you markers on  Sodium, potassium, albumin, alkaline phosphate, bilirubin, BUN, Calcium, Creatinine, GGT, globuline, AST, ALT, total protein, uric acid, LOH, glucose, cholesterol, HDL, LDL, VLDL, triglycerides, TC/HDL ratio and HgbA1.  We will have the In-Body available and optional for anyone that would like to have the test done. The benefit of completing your Biometric Health Screen and CHRA is that anyone that is currently on our insurance will be eligible to save $30/month on their insurance premium.  To receive the discount for the 2025/26 school year, you must follow the guidelines below:

     

    Complete the following between January 1, 2025, and March 31, 2025:

     

    Health Screening/Biometrics

     

    Health Risk Assessment (CHRA online questionnaire) available online starting February 1, 2025

    You may also use your general practitioner. For this, refer to the Physician Lab Form. This must be completed and submitted to UMR, dated between January 1, 2025, and March 31, 2025. Dates outside this window will not be accepted in place of the biometric blood draw.

                          

    Clinical Health Risk Assessment (CHRA)

     

    The health risk assessment will be open starting February 1, 2025.  It will close March 31, 2025.  Please go to www.umr.com  to take your CHRA survey.   

         

    The deadline for completing CHRA is March 31, 2025.  If you have not completed your CHRA on or before the deadline, you will not be eligible for incentives.  There are no extensions available due to time constraints of processing.  Please reach out to your building champion for help completing your CHRA if you need help.  

         

    EXEMPTIONS FROM SCREENS:

     

    Anyone that is pregnant – (Still required to take CHRA online)

     

    Anyone undergoing medical treatment that would affect the results of the biometric screens (Still required to take CHRA online)

    COMMON FAQ’s/Questions about the screens:

     

    • Do I need to fast?  Yes, for a minimum of 8 hours prior to the test.  You may have a glass of water.

    • If I’m pregnant, should I complete the screen?  No, you will only take the online Health Risk Assessment. You will need to email Wesley Lochhead to be added to the exemption list.

    • If this is my first year at JC Schools and I took the CHRA in August - October, do I need to retake the CHRA?  Yes, the CHRA and labs are to be completed to give you the discount and ability to participate in the $200 Wellness Reward for the 2025-26 school year.

    • If I recently had blood work done, can I use it?  Yes, if you have had a blood draw/workup completed after January 1, 2025, and before March 31, 2025.  You will need to have all other items completed as well and use the UMR Physician form.

    • What is involved with the screen?  Blood draw is the only requirement.  Please do not wear pantyhose or tights on the day of the screen.

     

    Please come at your selected time and be prompt.  The health screens are designed to serve up to 15 staff in roughly 30 minute increments.  If you come too early or too late, it will make the lines longer.

    You will fill out a consent and medical form stating that you give SSM the right to send your health screening information to UMR.
