  • Items Required for Kindergarten Enrollment:

    • Student’s birth certificate (state issued original, no photocopies accepted)
    • Immunization records
    • Any legal documents pertaining to the student (custody papers, guardianship documents, ex parte orders, etc.)
    • Parent/Guardian Photo ID
    • One proof of residency dated within the last 60 days that includes the parent/legal guardian's name and address:
      •      *Utility bill (electric/water/sewer/trash)                             
      •     *Cable/satellite bill
      •     *Internet bill                                                                           
      •     *Landline phone bill      
      •     *Signed housing contract                                                 
      •     *Mortgage Statement     
      •     *DFS or Social Services document
      •     *Homeowners or Renters insurance statement
